"you are a nurse doing an exam on a patient (camera is patient). you enter the room with hiccups, carrying a clipboard. you introduce yourself and explain that you have some questions you need to ask before you start the exam. the outfit doesn't have to be a nurse costume exactly. even if it's just something mostly white, that would be close enough. if possible, i'd love the top to be a little bit short so that i could see your belly button. . you ask many questions like the ones below, and after asking each question, you take time to write all of the answers down on the clipboard. (since you are standing while writing on the clipboard, try to support the bottom of the clipboard with your stomach, so that the clipboard might jump around with your hiccups). (try to work through this part slowly, this should be the main part of the video.). .you can ask different ones, or add more if you want, these are just some examples. .* how old are you? .* weight? height? .* how long has it been since your last exam? .* who is your health insurance provider? .* are you taking and medications? if yes, what medications? .* do you have any allergies? if yes, what allergies? .* do you smoke? .* do you drink? if so, how often? .* do you have a family history of mental illness? .* do you have a family history of cancer? .* have you ever experienced a seizure? .* are you sexually active? .* do you exercise, and if so, how many days per week? . after you finish asking the questions and writing everything down, you apologize to the patient about your hiccups. you say something like, "i'm sorry about these hiccups. i had them all day and i just can't get them to stop. i hope they don't bother you too much." . you then start the exam. (you can do any tasks here that might be something from a from a basic exam, these are some examples) .* maybe tell the patient to take deep breaths so you can listen. .* pretend to listen to their heartbeat (if you have a suitable prop). .* tell the patient to follow your finger with their eyes. then move your finger from side to side of the cameras field of view a few times. repeat this again but move your finger up & down instead of side to side. .* spend a little time just quietly examine the patient (maybe like you are checking their skin or hair for any problems) "
